INFOLINK'S Notice of Privacy Practices

This notice describes how medical information and other private information about you may be used and disclosed and how you may access this information. Please review this notice carefully.

You have privacy rights under state and federal law. These laws protect your privacy, but also require us to give information about you to others in some circumstances. We may tell you before we give the information to another organization when the law requires the disclosure.

Why do we ask you for this information?

INFOLINK uses the information to create an account that you may access at a later date. You may want to create an account to save a search or plan. You are not required to create an account. You do not need to use your own name or that of another person to register on this site.

Do you have to answer the questions we ask?

You may refuse to enter any personal information and still use INFOLINK.

With whom may we share the information about you?

Your name and other identifying information will not be made public and will remain private, unless the law requires us to disclose it. We will not share private information about you with anyone except The Montgomery County Foundation, Inc. which administers the site and will use the information in connection with that purpose.

Why do I need to be notified of this when I use INFOLINK.org?

We want you to know what we do with the information that we collect from you when you are using this database.

You have rights regarding your information.

• You may ask if we have any information about you and get copies. You will have to pay for the copies.
• If you do not understand the information, you may ask us to explain it to you.
• You may give other people permission to see and have copies of private data about you, including protected health information (referred to collectively below as "protected information"). We must have this permission in writing before we will release your information.
• If we have collected protected information about you, we may use it only for the purposes that we have listed in this notice.
• You may question the accuracy of any information we have about you by sending us your concerns in writing (to the INFOLINK Manager at the address below). Please explain why you believe the information is not accurate or complete. You may send your own information or documentation of information which we will keep and include with any information that is shared with another agency.
• You may ask us to share your protected health information with you in a certain way or in a certain place. For example, you may ask us to send health information to your work address instead of your home address. You must make this request in writing and we will fulfill it if it is reasonable.
• You may ask us to restrict uses or disclosures of your protected health information in writing. You must explain what information you want to restrict from being disclosed and to whom these restrictions apply. You may also write us to let us know that these restrictions no longer apply.
